By enrolling in the SKIN Bank Essential & Elite Membership at SKIN | Medical Aesthetics Atelier, you agree to the following:

• Minimum Commitment: Membership requires a 6-month minimum commitment. After the initial term, membership continues month-to-month unless canceled with 30 days written notice. Early cancellation requires payment of the remaining balance.

• Automatic Billing: Membership fees are automatically charged monthly to the card on file.

• By enrolling, you authorize SKIN | Medical Aesthetics Atelier to automatically charge your payment method on file monthly until canceled.

• Memberships are billed monthly on the enrollment date

• Monthly Rollover: Unused membership credits automatically roll over each month while the membership remains active and in good standing.

• No Refunds: Membership fees, banked credits, and benefits are non-refundable, non-transferable, and hold no cash value.

• Cancellation: Written notice is required at least 30 days prior to the next billing cycle. Billing continues until cancellation requirements are met.

• Expiration After Cancellation: Upon membership cancellation, any remaining unused credits must be used within 60 days from the effective cancellation date. Any unused credits after this period will automatically expire.

• Freeze Policy: Membership may be frozen for 30–60 days with written request and management approval. Freeze requests are not permitted during the initial 6-month term unless approved for medical or emergency reasons.

• Risk & Liability: You acknowledge that aesthetic and medical spa services involve inherent risks and agree to release and hold harmless SKIN | Medical Aesthetics Atelier, its owners, providers, and staff from any claims or liabilities related to services or membership participation.

• Services are subject to provider availability and medical appropriateness.

• Termination & Changes: SKIN | Medical Aesthetics Atelier reserves the right to modify membership terms or terminate membership for non-payment, policy violations, or misuse of benefits.

• Chargebacks without prior written notice may result in termination

• No Cash Value: Banked credits are non-refundable, non-transferable, and hold no cash value.

• Good Standing Requirement: Credits may only be used while the membership account is active and payments are current.

• Governing Law: This agreement is governed by the laws of the State of New Jersey
